In the right-hand panel of this page,
I have included a couple of forms along with information
that you may need during your daughter’s time here at SHA. One is for medication while here at school, both prescribed
and over the counter. The other addresses the need of a 10th
grade physical to comply with the Connecticut State Law.
Information regarding the yearly sport’s physical along with
the sport’s physical form can be found on the Athletics Web
page.
