SACRED HEART ACADEMY

HELPFUL HINTS FOR SENIORS

TRANSCRIPTS:

Your Transcript is a record of all your grades to date (Freshman, Sophomore, and Junior years) including scores from standardized tests that you have taken, and activities from the past years, that you submitted on your Activity Sheets.

In addition, we submit a Sacred Heart Academy School Profile and a list of your senior courses with each transcript.

How do I get one???

Transcript Request Forms are located on the table directly to the left as you walk into the Guidance Office. Completed Transcript Request Forms are submitted to Mrs. Donohue.  Be sure to:

  • Attach, with a paper clip, the Transcript Request Form and the transcript request fee ($2.00 per transcript for the

First six (6) requests and $5.00 for each additional request) to a large manila envelope 9x12), (addressed to:

Office of Admissions, college name and address).  Do not put your return address or postage on this

envelope.  You may write your name in the lower left-hand corner of the envelope.  Your completed application and

all required materials should be enclosed in this envelope.

  • In addition, if your college requires a faculty recommendation letter(s), it is your responsibility to request the letter(s) and to follow up with faculty.  Complete the Faculty Recommendation Information Sheet, which is available in the Guidance Office, and give it to a faculty member – the sooner the better. MAKE YOUR REQUESTS EARLY!! Faculty will submit letters directly to the Guidance Office. It is very important that you notify faculty as to due dates for letters of recommendation. The letters must be on file in the Guidance Office before you submit your transcript request and college applications.

You are responsible for checking all deadlines.  Please allow two weeks for processing.

SCHOLARSHIP APPLICATIONS can be submitted at any time using the same procedure as a college transcript request.  The fee for each scholarship is $2.00 (no limit)

First Quarter Grades will automatically be sent with your transcript once grades are available.  However, for transcripts mailed before NOVEMBER 1, 2007, it is your responsibility to request that Guidance forward your grades to those colleges. In these cases, please bring an addressed, postage paid, business sized envelope to Guidance.

MID-YEAR GRADES  (FIRST SEMESTER) WILL BE SENT ONLY UPON YOUR REQUEST

Keep a record of schools that require your Mid-Year Grades.  In January 2008, when these grades are available, it is your responsibility to request that they be mailed.

**The Guidance Department has a STRICTLY enforced deadline of Monday, December 3, 2007, for accepting college application and transcript requests that you wish to have mailed before Christmas Vacation. This deadline includes all colleges/universities with APPLICATION DUE DATES of January 1, January 15, or February 1, 2008.**

IF you have any questions regarding this policy please come to the Guidance Office.

Upon receipt of any college notification, please see Mrs. Donohue in the Guidance Office.  There are forms to be completed for this very confidential information.

Thank you.                                                                      (updated 8/07)
